We offer Memorial Plaques in honor of your loved ones for $250.

To order a Memorial Plaque, please fill out this form and send it to the office.

You will need the following information:

• Name of your loved one as it should appear on the plaque
• The Gregorian calendar date of passing
• The Hebrew calendar date of passing *if you do not know this, we will fill it in*
• Donor's relationship to the deceased
• Donor's contact information
